Yep have tried all the possible combinations mentioned in this thread, I see someone did a factory reset of the modem & the super admin username/password mentioned in this thread. Might try a reset on modem to see if that works.
Some people get missed in the upgrade process - often the gateway is offline, or not registered in our system. There's a newer version out now (B026), so I'd suggest if you're comfortable doing an upgrade yourself follow these instructions. Alternatively, call us up and somebody should be able to look into why you missed the upgrade last time and possibly help you do it.
